SUMMARY

This co-morbidity session included an RCT of rosuvastatin, pulmonary opportunistic infection impact on long-term lung function, liver disease biomarker validation, hepatocellular carcinoma risk factor assessment in HIV care, renal function after switch from TDF to TAF and showed gastrocnemius muscle thickness ultrasound measurement predicts sarcopenia.

HIGHLIGHTS

The Swiss HIV cohort described 2,407 people switching to TAF vs 1,113 continuing TDF; despite switch yielding eGFR improvement in those with renal impairment, there was no difference when baseline renal function was normal. Data from Latin America, Africa & Asia-Pacific regions showed, despite test availability, low routine HBV/HCV screening in Africa & infrequent hepatitis replication & alcohol assessment at all sites. A French cohort of people with HIV monoinfection showed, compared to transient elastography, serum biomarkers had good accuracy to detect NAFLD & high negative predictive value for advance fibrosis. After controlling for age/current smoking, history of pulmonary OI in 37/153 people was not significantly associated with breathing problems, symptoms, or abnormal spirometry. Rosuvastatin in people on stable ART at moderate coronary heart disease risk (Framingham 10-15%) with no other statin indication yielded no atherosclerosis progression benefit despite predictable lipid improvement and a trend to reduced hsCRP vs placebo.

CRITICAL ASSESSMENT

Serological tests provide an alternative to FibroScan for NAFLD & liver fibrosis diagnosis but with liver cancer as the 4th leading cause of cancer death globally, basic risk factor assessment needs improvement in LMIC. Renal benefits of TAF vs TDF may be restricted to those with lower eGFR, supporting use of ‘old tenofovir’ in appropriate populations. Although a mix of pulmonary OIs didn’t impact lung function long-term, extrapolate this small study carefully. Statins in people with no other indication didn’t impact atherosclerosis progression in people at moderate CHD risk, questioning broader statin use as primary prophylaxis in people with HIV.
